You make a fantastic point.
Even for those who say that sunscreen needs to be worn indoors (i.e for the rare occasion that you're working directly under a skylight), the amount of protection afforded by other objects obstructing the sun's rays is often incorrectly discounted. So for someone working next to a window with the sun shining through, they are unlikely to get as much sun exposure as they may think since the sky view in most cases is cut by at least half thanks to trees, other buildings, signs, and other objects in the environment.
